This website introduces Stacy Rodriguez, ARNP, and the services offered by ARNP Collaborative, LLC, the practice presented here as Concierge Collaborative. It provides general information so you can decide whether to contact the practice.

Becoming a patient
Browsing this website, sending an inquiry, or submitting an intake form does not by itself establish a clinician–patient relationship or guarantee acceptance into the practice. The practice will confirm whether it can accept you for services and explain the next steps for an appropriate clinical assessment. Eligibility for a treatment, prescription, or telehealth visit depends on individual assessment, professional judgment, applicable law, and the circumstances of your care. An in-person visit may be required. No particular prescription or result is guaranteed.
Getting in touch
The website, intake form, email, and text messaging are not emergency services. Do not wait for a reply if you may be having a medical emergency; call 911 or go to the nearest emergency department. Routine response expectations and any concierge arrangements are separate from emergency care.
Fees and outside services
A standard encounter is $75, or $150 total when additional diagnostics or labs need to be ordered or reviewed. Labs, imaging, medications, and other outside services may involve separate charges from the relevant provider or facility. The practice does not set those outside charges. Any individualized estimate and applicable patient rights remain separate from these general website prices.
Care already provided is nonrefundable, with exceptions considered individually. Follow-up for the same unresolved issue has no additional visit fee. Please give at least 24 hours notice to cancel a reserved appointment. Cancellations with less than 24 hours notice and missed appointments incur a $75 fee. These statements do not waive legal dispute rights.
